Social Prescribers: Thania Begum, Sabina Jaulim
GPs and Practice staff can refer patients for social prescribing when the patient requires non-clinical services. Link workers refer patients to the relevant services, based on their needs, such as Therapeutic gardening if they need company, due to situations of isolation and feelings of loneliness.
Patients that can be referred to a SPLW (Social prescribing link worker)
Anyone over the age of 18 who are looking for support for:
· Low mood mental health
· Isolation
· Bereavement
· Housing
· Employment
· Financial Help
· Physical Activity
